The Louisiana Region IV Science Fair is affiliated with Science Service, Inc., and

the International Science and Engineering Fair (ISEF).  This affiliation requires

that each student fill out specific forms.  They are described below.

                Required Forms Listed Below need Adobe

The following documents from Science Service, Inc., require Adobe® Acrobat

Reader  to be installed on your computer. If you do not already have Adobe® Acrobat

installed, you may download a free copy by clicking on the Adobe® logo below.

After the Adobe Acrobat Reader is installed on your computer, you

may view the forms in your browser window by clicking on the link or

you may save the forms to your computer by right-clicking the form

name then clicking "Save Target As" in the shortcut menu.

              FORMS Required by Science Service

         for ALL Junior and Senior Projects

             Science Service, Inc. (the organization that sponsors the ISEF and governs

             quality science fairs) has a Rules Wizard for FORMS that will help you

             determine which forms and approvals are required for your project.   Start

         with this wizard.   It isn't foolproof, but it is an excellent place to start

             figuring out which forms you will need for your project .
